The other terms don’t fit this idea. Chronic disease management refers to ongoing medical care for long-term conditions, not to conflicts between duties and personal gain. A citizen is simply a person within a community, and catastrophic describes a large-scale disaster or event, neither of which conveys the notion of duties being compromised by personal gain.
